Jonathan Azevedo has had a number of health problems in his life, including having open-heart surgery.
His dream to have his own skate park was granted through the Make-a-Wish Foundation, which arranged for the park to be built.
The Azevedo family wants to thank everyone who helped make Jonathan's dream come true.
